Harnessing the Strength of Home Care for Early Sepsis Recognition and Intervention


Sepsis as a National Health Emergency!  Commonly misunderstood as a hospital problem, the CDC and Sepsis Alliance report that over 80% of sepsis cases originate in the home and community.  Among the highest risk populations are the elderly, the chronically ill, persons with disabilities, medically fragile children, individuals with compromised immune systems, individuals with recurrent UTI and pneumonia, and others routinely within home care’s patient population.  Attend this webinar and learn more about how home care can identify the people who are most vulnerable to sepsis.


1. Learn details about sepsis as a health emergency, its prevalence and impact, and the imperative to address sepsis screening, education, prevention, early recognition and intervention in home and community.
2. Learn about an innovative new screening and intervention system for sepsis through home care and the elements and process involved in creating the tool.
3. Learn the screening tool, algorithm, protocol and patient education tool in detail, including how these are applied in specific case examples.
4. Learn critical steps in adoption and implementation of the tool at the agency/clinical level.
5. Learn the significance of this innovation for home care in the evolving  health environment, including: value based purchasing, managed care,  ACO, other integrated care and coverage systems, the Triple Aim, and health system reform goals (including prevention of avoidable hospitalizations/rehospitalizations and emergency room usage).
