Our home care and hospice agencies provide critical services to help our seniors, disabled and children remain independent at home with loved ones.

Our members provide the following services, but not limited to, certified home health aides, personal care, skilled nursing visits, wound care, companion care, live-ins, injections, medication case management, child care, emergency response systems, social work, bereavement, respite care, hospice, palliative care, behavioral health services, etc.

Study Proves Home Health Care Decreases Hospital Readmissions
Association shows how the Medicare Home Health services you provide can dramatically decrease rates of hospital readmissions. Not only does the study show a direct link between home health care and lower 30-day hospital readmission rates, but it suggests Medicare could save roughly $7 million annually just by ensuring those patients who receive a recommendation of home health care follow-through on their doctor's wishes.  Read the report here and press coverage from 101.5 here.

Starting next year, however, Medicare beneficiaries can have an added layer of security that their wishes will be known by their physicians and other health care providers. Beginning Jan. 1, Medicare will pay for advance care planning discussions with a doctor or qualified non-physician practitioner, such as a nurse practitioner or physician’s assistant.
