| Understanding DHSS Alphabet Soup-GO, ADRC, HCBS, etc.
|
Speaker: NJDHSS Representatives
Topic: Target Audience: Health Care Service Firms, Home Health Agencies and Hospices: managers, intake staff, social workers, community relations staff, nurses and others who need to understand these new community resources.
These initiatives will be rolled out to all 21 NJ counties as the state works to rebalance spending for long term care (LTC) and home and community based services (HCBS). This session will discuss the role of the ADRCs which are intended to be a single entry system for persons in need of home and community services. This session will also share information about (1) transitioning nursing home residents to alternative HCBS options, (2) initiating a fast track eligibility process in Aging and Disability Resource Connections (ADRC), (3) data about those counties currently involved in these initiatives, (4) services provided, (5) waiver consolidation and (6) plans for the future.
The State has already piloted these in several counties and plans to roll these out statewide beginning March 2008 so this information will be valuable to all.
This program is a follow up to the March 2008 program and will update participants on final decisions with the Global Options initiative.
Presented in half day session (repeated in PM) Registration begins for AM session at 8:30am. Session begins at 9:00 am. Session ends at 12:00pm.
Registration for PM session begins at 12:30pm. Session begins at 1:00pm and ends at 4:00pm.
