HOMESIDE HOSPICE

Homeside Hospice is seeking a Full Time Hospice Social Worker. Must be willing to travel multiple Counties in New Jersey.  Job duties include but are not limited to; responsible for the delivery of varied social work services to Hospice patients and their families. Provide initial psycho-social interviews and bereavement assessments, as well as direct casework services, community referrals, and bereavement support.  Education: Licensed by the State Board of Social Work Examiners, with a Master’s degree in social work from a graduate school of social work accredited by the Council on Social Work Education, and at least one year of post-master’s social work experience in a health care setting in accordance with N.J.S.A. 45:15BB-1 et seq and N.J.A.C. 13:44G.  A demonstrated ability in casework and counseling is required, including two (2) years of social work experience, preferably in work with the terminally ill population or a healthcare setting.  Must have current LSW, or LCSW. Maintains the standards of Social Work excellence in accordance with the principles set forth by all appropriate professional, regulatory and accrediting bodies. Performs comprehensive initial psychosocial and bereavement assessments. Conducts ongoing reassessment of patient/family needs. Provides short-term crisis intervention and individual and/or family counseling when indicated. Functions as a consultant and resource to the Hospice staff regarding patients/families’ psychosocial and financial needs. Homeside Hospice is looking for a Full Time Chaplain to implement pastoral care policies and procedures in conjunction with the social worker and bereavement coordinator. In addition to travel, the Chaplain must be available for On-Call hours during weekdays and weekends, alternating weeks with other Chaplains.  Job Description: Coordinates and integrates pastoral services with other patient/family care services in order to provide a continuum of care, including continuity of care following death or discharge. Prays with families and patients, if and when desired by the patient and/or family unit. Maintains communication and interest with appropriate local clergy, chaplains and spiritual counselors in order to promote a spirit of cooperation.  Education and Experience: Successful completion of specialized religious training, with a demonstrated ability to maintain a non-sectarian approach in the delivery of pastoral care is required, and at least two (2) years' experience in healthcare setting is preferred.  The individual must demonstrate acceptance of the philosophy and goals of Homeside Hospice.  Homeside Hospice is owned by nurses, operated by nurses.
