Patient self-management, based jointly on interviewing/assessment techniques and disease management tools, is especially important to the patient in the home health/hospice environment.  With the application of adult learning principles, patient activation and motivational interviewing, the clinician uses the OARS model and teach-back to enhance communication with the patient and reduce rehospitalizations.  The process supports communication connectedness to underserved populations.


  • Describe how patient self-management principles improve disease management.
  • Demonstrate using two motivational interviewing principles/models.
  • Demonstrate how to implement motivational interviewing and at least one additional self-management principle/tool.

Registration Fees:  Members $175  /   Non-Members $350

CONTINUING EDUCATION: 5.0 Nursing contact hours approved.

This continuing nursing education activity was approved by the Alabama State Nurses Association, an accredited approver by the American Nurses Credentialing Center’s Commission on Accreditation.  The speakers, content experts and nurse planner(s) for this program have declared no conflicts of interest.  There is no commercial support for this activity.
