The Home Care & Hospice Association of NJ is governed by a Board of Directors who are elected by our voting members and who represent the full scope of our membership.
Each year candidates for the Board of Directors are nominated by our membership and a slate is prepared by our member-elected Nominating Committee.

Vice President, Hackensack Meridian at Home

Christine is a registered nurse with 25 years of experience in home health and hospice.  Since joining Hackensack Meridian At Home in 2008, she has served in several key leadership positions for HMAH's home health operations.  In her current role as vice president of hospice, Christine oversees Hackensack Meridian Hospice's three locations  as well as VHS Hospice of NJ, a joint venture between HMH and St. Joseph's Health.

Mike has been a registered nurse for over 30 years. His passion is truly home care services, where he has managed Home Health, Hospice and Home Care services in many states, holding executive leadership roles such as Division and Regional Director, Vice President of Operations and Vice President of Quality and Standards.  He is the founder and President of MB Healthcare Consultants, LLC.

Monica’s nursing career began in acute in-patient and home health care in Essex County during the early 1990’s. Reducing health care inequities among communities and underserved home health patient population throughout NJ drives her passion to serve and lead. Ultimately, Monica strives to have a continuous commitment to her team, their patients, and the industry.

Regional Clinical Director, Preferred Home Health Care & Nursing Services

Trisha began her home care journey as an LPN providing PDN services. Today, she is the Regional Director of Nursing for Preferred Care.  She is also a certified consultant for CHAP and brings a wealth of knowledge in the Home Care and Private Duty Nursing space. As an experienced and passionate homecare nurse, Trisha is a strong advocate for ensuring nurses are paid fairly in home care.

Director, Holy Name Home Care and Hospice

Jatona is a passionate and dedicated nurse with over 12 years of healthcare experience in Home Health Care Services. She was promoted to the Director of Home Care at Holy Name Medical Center in 2020, which has evolved rapidly during these challenging times in healthcare. Under her leadership, Holy Name Home Care was recognized by the Jewish Standard Reader’s Choice as the First Place Home Healthcare Agency in 2021 and 2022.

Director of Nursing and Clinical Services, Center for Hope Hospice

With more than 30 years' experience in clinical settings which include inpatient ambulatory, rehabilitation, sub-acute, home care and hospice.  Heidi successfully expanded and implemented a new Palliative Care Program at Center for Hope Hospice and Palliative Care. She implemented a NEXSYS medication cabinet which allowed nurses to manage symptoms immediately when changes in medication occurred, and not have to wait for pharmacy delivery.  Today, Heidi uses her extensive experience to advocate for unmet needs for end of life facility based care, reimbursement and staffing issues.

Division President of Business Development, LHC Groups

Nathaniel joined LHC Group in 2018 through the merger with Almost Family Inc. As the Division President of Business Development, he is responsible for the growth, strategy and innovation of the North-East and Mid-Atlantic states which include over 40 separate agencies that complete over 300k home care visits annually.  Through the course of his professional career which has been dedicated to the care of our older adult population, Nathaniel has been a strong advocate and leader for aging in place. Nathaniel is a life-long resident of New Jersey and resides in Bergen County with his wife and family.

Tina Krajnikovich is the Founder and owner of Vitality Hospice and Palliative Care. She is an Adult-Geriatric Nurse Practitioner with a minor in Palliative Care from the University of Pennsylvania with almost 15 years of hospice experience. Tina started in the hospice industry after a personal experience and has made it her mission to provide high-quality care to those that need it most.

Northeast Area Director of Government Affairs, Bayada

Louise joined BAYADA Home Health Care in 2003 and serves as their Northeast Area Director of Government Affairs. She is a passionate advocate for frail and disabled children, adults, and seniors and the compassionate health care professionals who enable them to remain in the comfort and safety of their home. Louise understands the greatest way to educate legislators is by moving beyond the walls of the Trenton Statehouse and into the homes of BAYADA’s clients, allowing them to witness first-hand the powerful impact nurses and home health aides bring to New Jersey residents.

Toni Modak, MBA, BSN, RN, is a Vice President for Valley Home Care. Toni joined Valley as a Home Care Coordinator in 1993; since then, she has been promoted to key positions at VHC. Toni became a VHC Manager in 2002 and was promoted to a Director, overseeing VHC Admissions Services and Transitional Care, in 2012.

A recipient of the Tribute to Women in Industry (TWIN) Award and the Organization of Nurse Leaders Innovations in the Continuum of Care Award, Toni holds a Master of Business Administration from Capella University and a Bachelor of Science in Nursing from SUNY Utica.

Executive Director, Atlantic Visiting Nurse

Mary Pradilla is the Executive Director of Atlantic Visiting Nurse and has over 27 years of experience in the home healthcare industry, including the last 22 years at Atlantic Visiting Nurse. In this capacity, Mary is responsible for the overall operations of Atlantic Home Care and Hospice including daily operations, compliance with regulatory and accrediting requirements, patient safety and experience, team engagement, strategic planning, budgets, and forecasts as well as operating results. She also oversees Atlantic Private Care and the Adult Day Center.

Dani is the Founder and President of InstaCare Home Health Care. I started my career in home care at the age of 14, filling in for employees that would go on vacations over the summers and wintersHaving done that, I’ve had the opportunity to perform every role in home care, except a nurseAfter college, I became the Director of what is now called Jersey College and helped them gain national accreditationMichelle started working at Star Pediatric Home Care Agency 3 years after its’ inception in 2011, overseeing the financial end of the business, including the billing department, case management, as well as contracts In March of 2023, she officially became the CEO.  Michelle understands firsthand the challenges in staffing and would like to see nurses incentivized for working in home care.

Medical Director, Visiting Nurse Association Health Group, Inc. 

Dr. Sharma has been taking care of adults aged 55+ for more than two decades and has been boarded in Palliative and Hospice Medicine as a specialty for the last 15 years. Her primary focus is positioned around patient and family centered care for those diagnosed with serious illnesses. As the Medical Director of the Visiting Nurse Association, she works with home care and hospice nurses to increase awareness around the needs of patients and caregivers. Dr. Sharma is a board member of the NJ State Physician Orders for Life-Sustaining Treatment Task Force and the Conversation of Your Life Initiative.

COO, Promise Care NJ 

Jack is an accomplished and energetic Chief Operating Officer with over fifteen years of experience trailblazing in the Healthcare sector. He started in the DME/ Custom Rehab Mobility industry. Five years ago, Jack transitioned into the Home Health care environment. Presently, he is the COO for Promise Care NJ, overseeing and supporting his teams to provide quality care to referral sources and patients. Jack constantly strives for excellence when it comes to patient care and support.

CEO, Towne Kids

Mindy’s extensive experience in compassionate services spans 15 years and various leadership positions. After more than a decade championing critical services for families facing serious pediatric illness, she currently serves as CEO of Towne Kids, a pediatric skilled nursing homecare agency. Mindy is a devoted and passionate advocate for both clients and nurses, ensuring the success of each party and their relationship.
